REGISTRATION FORMS

Registration Fee April 1st through April 30: $175.00

Registration Fee AFTER April 30: $200.00

NOTE: Additional meet fees will be collected at the State and Regional meets for athletes qualifying and electing to advance to the next level of competition.

DEADLINE FOR ORDERING UNIFORMS IS MAY 5!!!!!!

Late orders may result in additional charges.

Registration fees DO NOT include price of uniform.

Purchase of a uniform singlet is MANDATORY.

Purchase of shorts, t-shirts, sweatshirts, and jackets, etc. is optional.

Uniforms are to be purchased directly from AWARDS LTD. To order your singlet (mandatory) and other club-related merchandise (T-shirts, sweatshirts, pullover, etc.) (optional), mail the order form with your check (no credit cards accepted) to the following address :
